New Patients
Your First Visit Starts Here
A short, practical guide so nothing about your first appointment is a surprise.
Before you arrive
Two things that save time
First, complete the new patient forms in advance if you can. Printable versions are available so you can bring them with you. If you are unable to complete them beforehand, you may complete them when you arrive, please allow time before your appointment to do so.
Second, write down when your symptoms started, what makes them better or worse, and what you have already tried. That short list makes the consultation far more useful.
