What to Bring to Your First Chiropractic Visit
A short, practical checklist for your first appointment at Peak Performance Chiropractic Clinic in Grand Junction.
Short answer
Bring photo ID, your insurance card, a list of medications and supplements, any recent imaging or reports, and comfortable clothing. Arrive 15 minutes early, or complete the intake form beforehand.
The checklist
Photo ID and insurance card. A written list of medications and supplements, including doses. Any MRI, CT, or X-ray reports you already have, or the name of the facility that holds them. Clothing you can move in.
If your visit relates to a collision or a workplace injury, bring the claim number and the adjuster's contact details.
Save time before you arrive
You can complete the new patient intake form at home and bring it printed. That usually takes ten minutes off the front end of the visit.
What the visit itself involves
A consultation about your history and goals, an examination including orthopedic and neurological testing, digital X-ray if indicated, and a straightforward explanation of what was found and what is recommended.
